The Performance Management Section has converted most of the performance evaluation forms to the fillable (.pdf) format. This allows evaluators to easily complete, electronically save and submit performance evaluation forms and supporting documents to the Performance Management Section. Please read the following instructions, FAQs and supporting documentation carefully before completing any employee evaluations.
